Making an Appointment

Please call Todd A. Moffatt, MD Surgery to schedule an appointment (254) 230-1234. Please keep in mind since Dr Moffatt is a specialist, some insurances require you to go through your Primary Care Physician.

What You’ll Need

Please arrive at least 15 minutes prior to your appointment and bring:

• Identification (government-issued, such as a driver’s license)
• Insurance Card
• Specialist copay
• Self pay patients – Please call before your appointment to make arrangements.
• Dr Moffatt will need complete information about your condition, so be sure to also bring all of these items that you have:
• Test results
• X-rays
• Mammograms
• Ultrasound
• Any other clinical information from your doctor or the emergency room. Feel free to contact our office to make sure your referring physician has faxed us your records.

You can save time by completing New Patient Forms in advance.

Cancellation

We request 24 hour’s notice if you need to cancel so that we may accommodate other patients.

Todd A Moffatt, M.D. Surgery will keep your primary care and/or referring doctor apprised of your condition and treatment.
